Please complete all fields. Speakers are required to have at least 20 years of experience in industry or academic (or a combination of both). The information on this form will be sent to the USFN Speakers Program Committee for their approval.
If accepted, your USFN Speaker term will be for three years. Once the initial three-year term is completed, an extension of up to three years can be requested.
